Due
to the liability issues, medical conditions & the aging of animal companions I have forms that are required
to be filled out and signed for each scheduled visit.
These
forms ensure I get all of the information I need from the pet owner and helps the pet owner to understand the services I provide
and the fees I charge for those services.
The
three forms that are required to be filled out are:

To finalize your pet care reservation request you will be emailed an electronic signature Client Service Agreement. Once I receive the completed form I will confirm your reservation and add you to my schedule.
There will not be a need for another consult unless requested as the information obtained during the initial
consultation, including the Pet Information Sheet and Veterinary Release Form, is permanently kept on file.
The Veterinary Release, and Pet information forms are also available upon request via electronic signature format.
Reservations
are booked based on availability so contact me as far in advance as possible.
